Are you taking the pill or IV? The pill carries much less risk.
Is your dentist experienced in managing bisphosphanate patients? If you don’t know, you should ask.
There is something called atraumatic extraction technique. Make sure you have a dentist (preferably an oral surgeon) comfortable and experienced with this technique. It is supposed to decrease osteonecrosis risk. Also I think there are recommendations surrounding which types of anesthesia are best. Anesthetic with vasoconstriction properties (shrinking of the blood vessels) are to be avoided. Sometimes they are necessary anyway because of bleeding risk.
Ask your dentist about antibiotics before and after surgery. To my knowledge this is standard of care.
From what I know a drug holiday from oral meds is generally not necessary. My research isn’t all that recent but from what I recall, it has been studied and not found to be of benefit as bisphophonates stay in your system for years. I’m not sure about IV meds

If you smoke, stop, it increases risk of osteonecrosis
