i think this will depend on how and who is paying the bills. i'm in the US and i have insurance - i have no doubt the insurance company who deny a claim that didn't give a reason for the hysterectomy.

i would even guess that the hospital would want a reason for the surgery in case of liabilities issues.

the hospital where i had my surgery required i get a letter from my primary care doctor stating that i am healthy enough and cleared for surgery. i guess they didn't want trouble if i died during surgery. i had to have cardiac, pulmonary, and chemistry tests to obtain the all clear for surgery.

Agree that insurance may cause problems, but that's not a reason not to ask. The hospital won't do the surgery unless they're sure that the insurance company will pay for it. (They know that if they do the surgery and then find out that insurance won't pay, it's unlikely that the patient will be able to pay their outrageous bill. Getting paid is their highest priority.)
