Thanks for the replies. I have an appt scheduled next Thursday with a local urologist who is a PA with 30 years experience. I most definitely want to end up with Mayo Clinic doing any cutting or biopsy, but I am not currently a patient. What is the process to become a patient? I envision the local appointment going with a DRE and possibly a course of antibiotics with a retest after the antibiotics. If the retest comes back still elevated I want to go to Mayo for any further testing. How do you do this?

Just call the Mayo Clinic and make an appointment. Your case is so early and so little has been done you can start from almost the beginning. You may be a lot of better off going to a center of excellence like the Mayo Clinic than going to a urologist. At Mayo, you have the option of many doctors to treat you, Not just a urologist. You want to speak to a surgeon that is usually a urologist, but you also want to speak to a radiation oncologist.
