From what I have read and experienced, lumbar sacrum plexus issues due to spinal canal and nerve root compression can cause pain/weakness/numbness from low back, buttocks, hips, down legs and to feet.
You could have hypermobility in joints like I do which adds to the problem because joints move more than normal. Did you have bilateral MRI of both hips/pelvis along with lumbar spine? What did the radiologist report list as impressions? You could definitely have a combination of lumbar and sacrum/pelvis issues contributing to your symptoms. Have you ever been diagnosed with hypermobility, autoimmune issues or connective tissue disease by a rheumatologist? Did you have EMG/nerve conduction study on lower limbs and do physical therapy over the years? Did it help any? Where did they do your cortisone injections and did they help any? Sometimes, joints can deteriorate with too many cortisone shots and not recommended for a long period.
My L4 was slipping over my L5 so they needed to fuse to provide some stability. In hindsight, I should have had surgery sooner in both cervical and lumbar spine to prevent worsening arthritis/degeneration and permanent nerve damage. I kept being told to delay, delay, delay since I was in my late 40s/early 50s but think I could have prevented some of the damage if done sooner.
