Answers below:
How often do you sanitize?
Every other day or less.

How many nebulizer sets do you sanitize at once?
One. I use the Pari eFlow mesh nebulizer. When I used the Aeroeclipse I had a bunch of sets and probably sterilized 3 at the same time.

Do you still soak in soapy water first?
After each use and before sterilization.

Do you leave the "extended" function on?
My machine does not have this function.

General observation: The frequency of sterilization seems to depend largely on psychological comfort, individual risk tolerance, and underlying disease state. In my case, I have bronchiectasis and asthma without any identified pathogen, aside from a dormant colonization.

@scoop hey scoop, you like Sue, have always provided excellent advice and insight, and I thank you both for that. How many times a day do you nebulize? I know most if not all nebulizer manufacturers state to do soapy water after using, but is that daily or EACH use? I neb 3x daily how can it air dry to reuse at 6hr or so intervals? Also I think it depends on the location of the area in the lungs of the BE. Mine for instance is both lower lobes towards the back, and right middle lobe. After I nebulize in morning, I do always get up quite a bit of mucus, then about 90-120 min delay more comes up. I’ve been told this is because of the lower lobe’s location make sense, as it takes time to move up to central and upper airway. If you neb more than once per day how does your equipment dry so quickly to use again after soap bath? Thank-you again for all you do for all of us. You also Sue.
