Oligo metastatic PC detected

Detected oligo metastatic ( low volume) prostate cancer on 11/08/25. Gleason score 8. PSMA PET shows Meta sized to Left testicle, left acetabulum , para iliac lympnodes, retro peritoneal lympnodes and right humerus. Started Bicalutamide on 19/8/25 and started ADT (leuprolide 22.5 mg) on September 4. Doctor advised to stop Bicalutamide and changed to
Aberaterone 1000mg with Prednisolone 10 mg yesterday.Planned to take another PET in December and start Radiation therapy. I am from Kerala, India

I'm sorry to hear your news, and wish you the best in your treatment

For my own oligometastatic prostate cancer, I received external radiation to treat the metastasis (on my spine) as well as radiation to the prostate itself.

I don't know if external-beam radiation will be practical for your 5 metastases — that's on the borderline between what they consider "oligometastatic" (low volume) and polymetastatic (high volume) — so it's possible your medical team will offer you a different alternative for treating the metastases directly, such as chemotherapy (Doxecetol) or radioligand therapy (Pluvicto).

Both of those can target cancer inside your body, even if there are additional spots that haven't been detected yet.

I hope your next conversations with your medical team go well.

good luck ...we are all fighting variations of the same...I wish you successful treatment and hope that in less than a yr, you look back and say, "wow, how did I get thru that ??..its all much better now !"

Since you are on both Lupron and Zytiga, your PSA may become undetectable. If that is true, then a pet scan will show nothing, Because your PSA is too low. That would be a good thing.

Do they want to remove the testicle that has the problem? Hopefully radiation will take care of your other tumors.

Your case sounds like it could be treated and you could have many progression free years.
