I thought about that myself - if it takes a year for T to return cancer it's not being feed how is that any different than the drug itself. Not being critical my hormone drug is 4k a month
That I don't know, because a year of ADT might keep PSA low for more than a year following. Prostate cancer becomes castrate resistant at some point, and then something other than ADT has to be used. My Gleason score was only 7, normally indicating a short term use of ADT, but the PSMA PET scan showed cancer in two adjacent lymph nodes, so it's being treated as a more aggressive cancer with longer ADT recommended. I know patients who have refused ADT due to the side effects and are taking their chances regarding longevity in order not to interrupt quality of life, yet. We make the best choices we can with the information we have.
@mccannr if your testosterone does not come back to even 50 for a year then it is equivalent to being on ADT for that year. That would be quite unusual. I was in a meeting today where one of the guys only had his testosterone go up to 70 after a year. He was not happy. Continue to have a number of side effects.

< 1 for a PSA is way too high if you’ve had surgery. It is recommended to have salvage radiation if it hits .2. If you’ve had radiation, then it can be fine if it keeps going down over time.
The amount of time you are on ADT depends on your Gleason score and your other problems found during your biopsy. If you have a Gleason 7 then 6 months is fine usually.
NCCN Recommends 18 months for a Gleason eight and 24 months for a Gleason nine. Other factors could change that.
