Yeah I think the cut off is around 75K for household income. I got copay assistance through my Urology practice. Didn't ask for it but turned out I knew the practice's Pharm manager and SSI had screwed up my Medicare so was in between coverage. The first bottle no information required the second and there on would require an application that includes all medical and financial information and a decision from them. Gratefully I only needed one to finish my treatment so didn't have to do a thing.
