Has anyone experienced profound hypotension with exercise while on ADT

Posted by mccannr @mccannr, Sep 12 11:18am

About 5 months into ADT (Orgovyx) I began to feel light headed during exercise, it was accompanied by a sensation bright lights and colors. I was able to take my blood pressure at the local Y and it was consistently 80/50. I was well hydrated, my normal BP is a little high (140/80). I think it might be autonomic dysregulation from low T. Anyone else?

Low blood pressure directly affects your Feeling lightheaded with or without exercises.

Yes, low testosterone can be associated with autonomic dysregulation, particularly an unfavorable shift in cardiovascular autonomic tone in men, and low testosterone has been linked to a higher risk of autonomic dysfunction, though the exact nature of the relationship is complex and multifactorial. Studies suggest a bidirectional link where low testosterone and autonomic dysfunction can influence each other, often in the context of conditions like metabolic syndrome.

Here is an article that discusses the above in more detail.

Thanks. Those studies discuss low testosterone, but not the essentially zero testosterone of ADT. So far I have no evidence of glucose intolerance or metabolic syndrome. I wonder if anyone has looked at CAN in ADT patients. Looks like the mechanism is too complex to suggest an intervention on my part.

ADT doesn’t drop your testosterone down to zero. The reason they give people Zytiga is because ADT only gets it down so low and Zytiga gets it down below five.

After being on ADT for six years, my testosterone never got much below 5, except when I was on Zytiga.

Are you sure yours is getting down to zero?

What's strange with me before chemo I was on BP medications and now I have to be careful for my BP to be too low. Don't know if this helps or not.

Must admit, That is a little unusual for ADT alone. Most people need to take Zytiga To get that low.

Everybody’s system is different.

I did after radiation but it turns out it wasn't the Orgovyx. I was taking Flowmax (lowers BP), Viagra (lowers BP) and Metoprolol (lowers BP) simultaneously and it was too much. The doctor replaced the Metoprolol with Ativan, and I was OK.

That’s really strange, Ativan’s a narcotic, And they really tightened up on prescriptions for it in the last couple of years. It’s not really for treating blood pressure issues.
I take metoprolol to Keep my heart rate From speeding up. My wife takes Ativan occasionally for anxiety, they won’t give her prescriptions often because some people can get hooked on it.

The Metoprolol was given to me off label for anxiety that was also causing a lot of heart palpitations, not blood pressure. Early in my treatment I was constantly having minor Afib episodes because my anxiety was through the roof. My GP thought a beta blocker would be helpful because it would slow my heart down and reduce the physical symptoms and, for the reason you said, because it's not a narcotic. I was only on it for 60 days, 30 days at 1 mg and then 30 at 1/2 then didn't need it anymore.
