Thinking I Have Malabsorption

Hope someone could shed some light on this. All I’m told is I have anxiety and depression. Which I do for years. Constant heat in esophagus. I take Nexium. I’ve had endoscopy and colonoscopy. Found a polyp in stomach. It was removed. Colonoscopy was fine. I have no appetite though I get hungry. I have episodes of constipation and diarrhea. In April 2024 I was experiencing abdominal pain. Went to ER. Did a number of tests. CT, MRI, blood tests. I had 2 small tumors in bladder. Was bladder cancer. Those were removed and treated. Had a uterine polyp which was removed. But this lack of appetite and being hungry at the same time, the nausea, and heat in my esophagus is killing me. It doesn’t matter what I eat. I hope I explained as much as I could. I’m really upset about this. I’m 68 yr old female. Since the pandemic I’ve gone downhill. I didn’t have Covid but it affected me deeply. I don’t want to do anything. Plus now these physical ailments are making me feel worse. I’m not sure if I have malabsorption or something else. Sometimes I think I have PTSD because of the past 4 years. I’ve lost a lot of weight but gained some back. My PCP just did a blood test for celiac and it was negative. I explained as much as I could. This post may seem confusing and I’m sorry. I finally got a stool tested and it is high in fat.
No blood, white cells, h pylori, and the fecal elaste was normal.
Im having the run around with this clinic so I decided to try and internal doctor and see what he thinks.
I have a weird gut history, too long to go into. But for a few years ive noticed these fatty stools come in phases and now they're just a bother.

Not really sure the cause but Ive adjusted diet and exercise, etc. I did fid out I had a vit B12 deficiency around three years ago, also diagnosed with idiopathic polyneuropathy. So, something with small bowel? Intrinsic factor? Gallbladder or bile duct issues?
No ibd. I honestly think it's fat malabsorption.
So im praying this new doc will listen.
Ive dealt with chronic infections and that might be a part of it.
Whatever it is, I will try anything to help it!
Just dont want to be blown off again!

Hi. Sorry to hear you feel like you are not being heard at your doc’s. I’ve battled gut issues for quite some time so I’ll share what helped me. I have no medical training - just patient experience. Two meds I use are Zenpep (or you could try Creon) to aid with digesting fats and Ondansetron (generic for Zofran) for nausea. I do suggest you have your gallbladder checked. It may look fine on a CT Scan but a HIDA test will tell if it’s working. I’d research a lot before having it removed though, as that can come with its own host of problems.
Could you be having bile reflux rather than acid reflux? There are meds to help with that such as Colesevelam or Colestipol (sp?).

Hi thank you. I don’t have a gallbladder. Zofran doesn’t help. Plus I’ve been scoped a couple of times. I’m thinking it’s not bike reflux. Thank you again for your response.

Talk to your physician about EPI. I had to insist on another blood draw specifically for pancreas issues. Have had this for 3 + years. On enzymes now at $647 per renewal. I’ve got to build my muscles back. Good luck

Currently on Creon RX for EPI. Have to take it every time I eat anything. 1 with snack and 2 with meal which is sometimes hard to define. I usually eat multiple small meals/snacks throughout the day. Creon was helping quite a bit but I'm back to diarrhea today when anything goes in. I have Celiac Disease - diagnosed about 20 years ago when in my mid 50s. I'm very careful about reading labels, don't eat out much and make a point of telling server I must eat gluten free. Sometimes I still get something contaminated that makes me sick. However, getting back to the EPI issue, I don't know if I'll be taken off the Creon and go back to short term Medrol dosepak which helped somewhat a few months ago. My weight goes down about 4- 5 pounds when the diarrhea hits but back up when I stabilize.

I am just plain jealous (in a good way) of all of you that don't gain weight with absorption/💩/pancreas issues. I am struggling to lose any ounce I can. I have Celiac disease, I was diagnosed at 50, a lifetime of damage to my villi, so I still don't absorb well. I know weight loss for the wrong reasons isn't good, but I'm still jealous 🤪ShelleyW

I understand. I had been overweight for most of my adult life I always tried to avoid fads, but was constantly on a diet. After years, COVID came and my husband had additional health issues that essentially kept us housebound for 3 or 4 years. I loosened up on dietary restrictions so we'd derive more pleasure daily. To my great surprise, I didn't gain weight. I actually had weight loss without trying. I attributed it to the stress of caring for my dying husband. It turned out to be malabsorption. At one level I didn't care. I was thrilled to weigh what I had in my early 20s. Ironically people who had known me for years would say how good I looked while my hair was thinning, my nails were breaking and I thought I looked like "death warmed over." Luckily my weight stabilized when I started on PERT.

We are told that the symptoms of EPI vary. But it's hard to understand how some can't gain a pound and others can't loose a pound.
