Follow Up Treatment for Evenity Injections

Has anybody tried Evista or any other SERM as their follow-up treatment for Evenity? I just received the 7th Evenity injection, and I need to decide my next step. For most people, an antiresorptive medication is recommended as a follow up treatment, and SERMS are antiresorptive. However, I cannot find any studies that support using SERMS instead of Prolia or Bisphosphonates. Most doctors recommend going with the stronger drugs. However, I would prefer using a drug that has less side effects.

Three CTX tests were in the 500s ((Normal Range: 104 to 1008 pg/mL). One was 630 and one was 886 pg/mL. All while taking raloxifene. Same here on the studies - only done for Forteo or Prolia and not Evenity.

Thank you for sharing, I was just about to ask.. The reference range of CTX is pretty broad, and lots of people still fall within "normal" whether they are on treatment or not. My endo likes to see CTX below at least 300 while on an antiresorptive, but I've read some aim even lower.

Just curious, have you noticed any pattern between your CTX levels and whether your bone density (or Tscore) stayed stable? 630 or 886 definitely sounds like the higher end. Was your baseline CTX (Rx-naive, or without any prior treatment) pretty high ?

I started treatment for osteoporosis with an endocrinologist in 2011. At that time and with that doctor, my test results were not available in a digital app. He gave me paper copies of the results of my bone density scans but not the results for the blood tests. I should have asked for the blood test results, but I didn't. So, I don't know if I had a CTX done before August 2018 when I switched doctors. My 2018 CTX level was 521 (Normal range: 104-1008 pg/mL). Once I started treatment in 2011 (first with HRT and then Evista), my T scores improved. I haven't noticed any pattern between my CTX levels (because I don't know the earlier results) and my T scores, but there probably is one. The higher CTX levels (886 and 630) occurred when I wasn't exercising very much. That situation changed and the next result was 514. my most recent CTX level was 208 with a different normal range: 34 to 1037 PG/mL. This test was done after 5 Evenity injections.

My dr wants me to do Reclast after finishing with the year of Evenity shots. I’m supposed to call to schedule and I’m so hesitant!

@janeted May I ask why you are feeling hesitant about following your doctor's recommendation for Reclast? What did your doctor about the reasons for a Reclast infusion following a year of Evenity?

I did 12 months of Evenity injections. My bone density scan was repeated after the Evenity and I made significant gains in both hip and lower spine following the Evenity medication. My endocrinologist explained that while Evenity helps to stimulate bone density that increase does not maintain over time. Reclast helps to prevent bone loss and maintain bone density. I then followed by endocrinologist's recommendation to have the Reclast infusion.

I did not have any reactions to Evenity or to Reclast. I expected to feel flu-like symptoms and fatigue after the Reclast infusion as these are the most common side effects but I did not have those symptoms.

Dr said I had improvement in spine but not much in hip after a year of Evenity. She said the reclast will lock in the gains I made since Evenity doesn’t last after the year of injections. I guess I’m hesitant after reading all the negative experiences with bone pain….also I couldn’t get a straight answer as to why reclast and not Prolia shots….🤷‍♀️

Thanks for sharing! Was the higher CTX level (630 or 886) linked to a drop in your T-scores?

The recent CTX 207 reading seems in line with your Evenity treatment, at least imo, since Evenity is dual acting - it lowers resorption while boosting bone formation, so that makes sense.

Does your doctor know why the injections didn't work? Did he/she work out an alternative bone regeneration plan, using anabolics and/or bisphosphanates?

I ask only because I'll be starting injections pretty soon. I'm wondering what to expect; I didn't realize that some people don't get a therapeutic effect from these meds.
