Just Finished 18 mos on Tymlos and....

I finished 18 months on Tymlos on Feb. 25th, 2024.
I'm very disappointed with my follow up dexa results.
4.8% worsening of left hip
4.2% worsening of right hip
5.2% improvement of lumbar spine
My Dr. says I definitely grew bone back even though it doesn't show.
I just don't understand.
I'm going back on Actonel monthly.
Anyone have any imput?

Best not to put your health care in the hands of a doctor/PA who is not specialized in the field you need.
It took some shopping, but I'm happy that I found a specialist who studies bone health only!!

We have healthcare advances and new treatments every day. Hopefully, in years to come, our daughters and granddaughters, will avoid this illness.

My Tymlos treatment will end in April the latest. My options for after are Reclast or Fosamax. I'm hesitant to do Reclast because it's an infusion and there's no going back.

I've been advised that I could take Fosamax for a year, and then stop for a while. I'd monitor progress with DEXA scans and CTX tests. If needed, I could then go back on. Meanwhile I'd keep paying attention to nutrition and exercise.

My T-score was -2.0 six months ago. It's probably even better now. That gives me the luxury of not having to take another big bone-building drug later on.

I have heard of the "holiday" but am leary of it.
My bone doctor said my sudden drop in density was caused by stopping prolia in preperation for a dental implant.

I was being treated by my GYN and was told by her PA to go off the Prolia before the dental work, then get my next shot immediately after. I was due for my six month dose, a few weeks after the dental work. In that short time my bones
weakened dramatically. A trip to the ER showed a fracture, I had surgery on that one. Then four more fractures immediately after.

For that reason, I'm petrified of a "holiday". But I cannot stay on the Tymlos longer than two years.
I now see a bone specialist and am anxious to know what treatment she advises post Tymlos. I've heard reclast can be a once a year drip or twice a year injection. I'd love once a year. Is that for the rest of your life or is there a maximum time for Reclast also?

I had a horrible rebound effect from stopping Prolia, too, and not being put on something stronger than oral bisphosphenates to counteract it. That’s one you can’t take a “holiday” from without a plan for another medication. I’m not sure about the maximum Reclast dosage or whether it’s available as an injection, I was under the impression it was only as an infusion. I’m glad I know my doctor now will order labs to keep a check on my BTMs, no matter what I am on or not on. Best of luck and health to you!

I see Dr Christine Jablonski who is with Orlando Health and the Jewett Orthopedic Center. She is very hard to get in to see, was not taking new patients for a while but I understand she has a PA now to assist her. We need more like her for sure. Her mom went through osteoporosis fractures and it lit a fire under Christine to do more for bone health.

Thx so much for the info. I just had a consult with Dr McCormick - he's so knowledgeable but I need an additional batch of tests. So I doubt my GP would help order them and definitely not my endocrinologist's office. i recall once asking @ Windy Shores how she managed her working with several different doctors on her osteoporosis and she had replied that they were each unaware she was working with others. I may have to order all the additional tests myself and pay out of pocket. My other options are to find and work with a competent bone specialist in FL or find a new GP who would be open to ordering Dr McCormick's tests.

How can your doctor tell bone grew back? I dont understand your scores unless bone grew back but gained no density, but I don't know how that could happen. I think I would get another opinion. Is your doctor a bone specialist?
From my experience only a bone specialist is well versed on osteoporosis treatment. An arthritis doctor is a bone specialist...

I did FOSAMAX for a short time. It gave me terrible indigestion/ heartburn/acid reflux.
My sister in law took FOSAMAX also and had terrible indigestion. She hasnt taken it for years but still has indigestion and needs to be scoped once a year to monitor the damage to her esophogus.

I just recently started taking Fosamax.I hope I will not experience negative effects from it. I continue taking Citracal and eating calciium rich foods.

wait and be patient. Be sure you are taking your DEXA on machines offering TBS software analysis!!! Otherwise you can't compare exactly and so you might be disappointed unnecessarily! If your doc said you DEFINITELY grew bone back, what is there to be disappointed about? Give it time and hopefully you are taking both kinds of meds (bone density and bone reabsorption) and give yourself a 5 year horizon! And DON'T give up! Talk to your bone specialist doctor!
