I was on Zytiga For 2 1/2 years. For some people, it can be quite difficult. It gave me high blood pressure for which I now have to take three pills twice a day, it gave me four afib Events (had not had one for 17 years), The third one putting me in the hospital for four days. I have been on Darolutamide For two years and it has worked much better without any side effects and kept me undetectable.
Talk to your doctor about your kidney issues and afib danger ask him if you can get on a different drug. The only other choice is one of the lutamides.
I am on a blood thinner because of afib. Prevented me from having a stroke when I was in continuous afib for 3 1/2 straight days. Not sure if you are on one, but it might make sense to ask your doctor if you stay on Zytiga.
The problem may be that all of the drugs have at least some effect on the kidneys. Darolutamide Has the least side effects of the lutamides, So it’s a good choice. I know a lot of people on it that are quite elderly and it’s working great for them. Many of them have dropped ADT and are just on Darolutamide. Apalutamide doesn’t appear to affect the kidneys much either. Enzalutamide seems to have a slightly greater risk.

While some patients may experience mild, non-significant changes in kidney function tests, those with pre-existing kidney disease may require closer monitoring or dose adjustments.
