How long does your oncologist have you on ADT?
I had surgery in March 2020. I was undetectable for 3 years, then started to see some PSA. .05.....06 ... Eventually climbed to .25 in Feb/2025, which triggered follow up treatment, starting in Late March 2025. Orgovyx for 6 weeks then 25 radiation treatments which ended 6/25/25. I am scheduled for ADT for 6 months,ending around Oct .1. My question is- is 6 months the norm?

YES, that is my issue 'fatigue' I'm currently on 5mg twice a day...

18 - 24 months, have been on Orgovyx 3 1/2 months so far. They told me this is the norm of what I have (Gleason 8, and I have some higher risk patterns).
Stage 4 with lymph and bone mets, Gleason 9 diagnosed at age 74. Started Digarelix injections with Abiraterone/prednisone orally. Jan 1 Rad Onc switched me to Eligard injection for convenience (every 3 mo instead of monthly). My body doesn't seem to like Eligard (Lupron) with Abiraterone and on July 1, 2025, I was due for my third Eligard injection and my Doc suggested I take a "vacation" from ADTs altogether. I was on ADT for 3 years and tolerated it pretty well until the switch to Eligard which drained my energy levels and brought pain in glutes and hamstrings. I assumed I would be on ADT the rest of my life but my Doc convinced me that "vacations" were now a thing. So we shall see! (pain in the rear seems to be fading away and energy levels are coming back up)
My radiation oncologist told me that the standard based on my numbers would be Lupron for 24 months. I had an RARP in 2023 with recurrence in 2024. My initial Gleason was 8 but after the pathology report it was increased to 9 and my Stage to 3. I started Lupron on December 3, 2024, three months before he would start my 39 radiation sessions. I get the Lupron injection every three months with my next one in early September. My biggest side effect are all the hot flashes. My wife recently bought me a neck fan, which has been a great addition. As soon as a hot flash starts I put it on and it blows cool air around my neck and head. It has been a game changer for me. I think it was only around $20 on Amazon and is rechargeable.

I had Gleason 9, Decipher score 0.92. My urologist prescribed 24 months of Elegard in addition to 44 photon radiation treatments. PET scan showed no spread of cancer outside the prostate. I took the first two (6-month) Elegard shots, but on my appointment for the third shot, I told him I didn't want any more ADT. To my surprise he said something like "that's OK, we got you two shots, PSA is staying low, so we can skip the last two". I was thrilled because I had bad side effects from Elegard - night sweats, hot flashes, penile and testicular atrophy, and worst of all the mental effects - depression, insomnia, anxiety through the roof. I felt like I was going crazy. Only exercise gave me any relief. I am now one year off of Elegard and PSA remained "< 0.02". I asked him if I could get ,my testosterone back, and he said Okay let's try Clomiphene. I took Clomiphene six months in all, T level only got to 39 despite increasing the dose midway through. After that I asked if we could try hCG, which he had originally disliked, but he said OK and I started injecting hCG once a week. If that didn't work he would go with topical testosterone. My last test showed T level of 537! My symptoms are improving, though I still have some hot flashes. My confidence is better, anxiety is gone. I am getting some erections too, not as good as before, but still great. He will try to wean me off of hCG, which he saw as a jump-start to restoring my natural production. Last week I only took 2000IU instead of the 2500 IU I had been taking. We'll see how it goes.

In the U.S., you might still be able to fly under BasicMed as long as you qualify for a driver's license. Here, Transport Canada told me I'm not likely to be allowed to hold any license or permit again (even the Recreational Pilot Permit, our equivalent of BasicMed), so my wings are permanently clipped.
Surprisingly, it was the spinal injury that grounded me (which my orthopedia surgeon think was bogus), but I know that if I beat that, they'll just bring up the ADT+Erleada that I'm on permanently, so it doesn't seem worth spending $10K+ in legal fees and a year or more of my life to fight it, even though the aviation lawyer recommended by COPA thinks I have a decent case and could set an important precedent.
Might depend on your age and financial status . I m 76 and retired now from being a bus driver for 12 years. I was an ATC for 4 years and got fired by movie star Reagan, that bad event of course led me to a better paying job, less stressful too at a nuclear plant for 21 years, bus driving came after this. Moral as discussed before- bade things can lead to better things. The US Govt. Screwed itself losing all that talent: just like what’s happening today.
