Prostrate cancer in lungs that are subcentimeter in size?

Posted by jester2022 @jester2022, Aug 15, 2023

My prostrate cancer is nowhere in my body except in my lungs per three Pmsa scans. My psa is 20.1 and has gone up 1 point in a year.
The size of the nodules are subcentimeter in size. Very small.
Should I start Lipton treatment or wait till they are larger?

My Stage 4 Prostate Cancer spread to liver and lungs. Diagnosis in 12/2020, Radiation, Chemo, (2021) + Hormone continues. 3.4 years in overall health is good, maybe best ever. Go for CT Scan next month.

PSMA PET scan in 2/24 showed a small lung nodule. Follow-up 6 month CT scans in 8/24 and 1/25 show the nodule still there but it has been stable (4mm). My Drs have not expressed concern and the radiology impression is that it's probably benign and probably infection. But if it's "infection", wouldn't it go away by now. I've got another 6-month CT tomorrow. My PSA has been < 0.01 for 2 years. But, I've learned that neither PSA or PET are always reliable. I've read it's highly unlikely to have metastasis to the lungs without it already showing up somewhere else first?

Several years ago I read something about prostate cancer in the lungs, but it was never a stand alone and had always metastasized from somewhere else and most prostate cancer in the lungs was only discovered post mortem. The only (more favorable) way to determine if these sub centimeter lung nodules are cancer would be via a needle biopsy not a PSMA. My lay opinion is that the nodules are not cancerous and I would not begin cancer treatment until several of the nodules were subject to a biopsy.
