GERD issues

Posted by janarose @janarose, Feb 29, 2012

I am 22 years old. I don't smoke or drink. Growing up I can remember having a slight hoarse voice in the morning if I ate to late at nights. But never anything too miserable. It would go away within a hour after drinking water.
March 2011 on a Wednesday. I woke up with a hoarse voice that never went away. After a few weeks I started to get slightly worried so I went to my family doctor. I went 3 times over a course of 5-6 weeks and they had no idea after giving me multiple steroids and antibiotics that did nothing. I also had no pain in the beginning my throat didn't hurt at all. My only symptom was the voice. So they sent me to a throat specialist. (Audiologist) He did a throat endoscopy and told me I have GERD. He prescribed me omeprazole 20mg once a day. I changed my diet. I stopped eating hours before I went to sleep. I have never been a soda drinker, I am not a fan of fatty foods. The fattiest thing I like to eat sometimes is chocolate.. So I gave that up. I don't eat big meals. I have always been a snacker. But I didn't seem to get that much better so back as planned in 6weeks. He told me to start taking it twice a day. my throat started to hurt and become really sore around this point. I would wake up in severe pain, and it would hurt through out the day. I would take 2- 20mg omeprazole and I would also drink baking soda mixed with water whenever I couldn't handle the constant burn anymore.. I wouldn't drink the baking soda/water at the same time as taking the omeprazole. I made sure I did that hours apart from one another.
So about 10-12 weeks later my symptoms got much worse. It wasn't working. Nothing seemed to work.
So I went back to the doctor. They did another endoscopy and my throat is more swollen than the first. It's starting to swell up even higher around where my thyroid is.
I started having chest pains and it has become hard to swallow anything.
My heart rate gets insanely high in random occasions.
I feel a knock/thump whenever I swallow.
My throat is always hurting.
I am nauseous when I eat.
It's painful to eat or drink anything; I have lost almost 30lbs because of it.

So I have had a endoscopy twice now,
I got a barium swallow done and he told me I have acid reflux. probe test done,.,Blood work....How many more test need to be done?
I haven’t heard my voice in a year now. It's to the point where I am becoming depressed and I ignore talking to people. I ignore hanging out with friends, because if I talk to much my throat becomes incredibly sore to the point where I won't be able to sleep. I can't yell or whisper. I have one tone. If I yell then I’m pretty much out for the rest of the day because my throat hurts so badly.
I am thinking about going to the Mayo Clinic. Has anyone else been through this? Do you think they will be able to help? Could it be something else?
Im desperate.

I am really glad I found your story. I’m sorry you’re still having issues. I am having the exact same issues to the T. I’ve seen a heart doctor who is starting to run tests. I also did a PFT which said I have asthma even though I’ve never had any asthma before in my life. I see a pulmonologist in like a month. I have to take omeprazole for two more weeks before I can see a GI. All the heart tests so far have been good. So is my blood pressure. I’m frustrated like you but glad I’ve stumbled upon this.

I’ll be interested in your experience and recommendation from docs. My last appointment was with pain management. He couldn’t figure out, based on MRIs, what could be causing symptoms other than neck/arm pain. Gave me script for low dose Gabapentin with vague dosage instructions. “1-2-3 times a day” to see what works. I haven’t taken any yet. Just don’t see the point.

I have moderate GERD. I am using omeprazole 40mg 2x day, but I need more help with acid coming up my throat after I eat. The sour taste in the back of my tongue is very bothersome. I read that decaf ginger tea may help. What brand do you recommend? Does it help with stomach acid?

Hi there - I found something helpful for that problem - a supplement made by "Life Extension" called Esophageal Guardian. You take the capsules after (or before? can't remember! The instructions are on the container). The contents form a protective layer overtop of the food contents of the stomach, and they can't back up and irritate either the stomach or the esophagus. I was surprised at how effective it was. The active ingredient is "Alginic Acid" - not sure how it works but you can look it up. By the way, although I found this very helpful, I have no interest in the company or the product - other than how much it helped! Good luck!!

Sorry this is so long, but I wanted to get everything out there.
At end of December after a heavy meal I woke to indigestion and stomach pain. Began as gnawing in lower stomach then upper and continued daily. Started 10 day course of Prilosec. Had similar issue a few months before and was prescribed Prilosec and it resolved. Prior to that, about 2 years before, I had ended up in ER because in addition to indigestion, I had pain in my upper chest and left arm. No cardiac issues; stress and GERD were diagnosed.
Early January I had a telephone visit with PCP. Some food triggers, worse on empty stomach and upon waking. Not always stomach pain; indigestion with spread to shoulder and arms. Sinus-like pressure in face. Mild cough with indigestion hours after eating. Occasional hot flashes/cold sweat and overall feeling unwell. No appetite. PCP prescribed continue Prilosec; second course if needed.
After 3 weeks, still no better, so saw another doctor in PCP office. He ordered blood work and prescribed sucralfate. At the same time, I was trying to get an appointment with a doctor in the digestive health area, but was told next appointment was 6 weeks out. Finally got a video appointment with a gastroenterologist. No interest in blood work; changed to liquid sucralfate and scheduled an endoscopy for 6 weeks later. Could not tolerate sucralfate so was switched to famotidine 40 mg. 2x day.
Blood work showed high Gamma GT and elevated CO2. Emailed PCP who said since liver enzymes looked good would not normally have checked gamma GT, but would order liver ultrasound if I wanted. I have had liver enzymes show as elevated in past blood work on and off over many years. Liver ultrasound once, but nothing showed as abnormal.
Endoscopy showed small ridge at end of esophagus appeared mildly thickened, so ordered fluoroscopy esophagram. Biopsies of tissues were normal. Esophagram results were normal.
I can go for a month or two with no symptoms, but when they start they continue for many weeks. I have been on famotidine continually since mid-February. I consulted a nutritionist to get some ideas on how to change my eating habits. I wondered if the fatigue was due to lack of protein, etc. One thing she suggested is DGL before each meal and that seems to help with stomach pain.

What bothers me are the symptoms that I don’t see as being typical GERD symptoms:
indigestion pain spreading into shoulders and arms; sinus-like pressure in face and foggy brain; fatigue; hot flashes with quick wave of anxiety; feeling like I need to concentrate to breath, but having no trouble taking a deep breath or exercising; bouts of severe constipation. I have Hashimoto’s thyroiditis, but it is controlled. My latest routine blood work came back fine, including test for Celiac markers. So frustrating that I keep being told my blood work is good — but I’m not feeling “good”. Just don’t know what to do next.

Be very observant with the side affects from the gabapentin and talk to your doctor, if necessary.

Hi @tommyastin, and welcome to Mayo Clinic Connect! Thank you so much for your helpful comment.

If you are comfortable sharing, what symptoms are you most familiar with, and how are you doing now? You mention having them while taking a Pantoprazole and Hyoscyamine. Are you still taking them?

I have Gerd too which also caused my Barrett’s esophagus I’m sure. My doctor prescribed 40mg of Omeprazole a day which helps control it. I always eat 2-3 hrs before bedtime if I don’t and sleep on my side here comes Gerd. You should talk with your doctor about maybe getting scoped? Best of luck to you!
