Has anyone noticed a relationship between a herpes outbreak and exacer
I have chronic low back pain which radiates to my right buttocks. I have been seen by pain specialists for many years and had epidurals and radio frequency ablations in the past. These have either not been helpful or not lasted longer than a year or 6 months. However, I also have burpees outbreaks that occur on my buttocks approximately every 3 to 4 months. This clears with antiviral. I am not taking the antiviral for suppression at this point. Has anyone noticed an increase in their low back pain or buttocks pain?

Herpes outbreaks can be aggravated and triggered by many things and the virus lays dormant, hiding in your nervous system for life.
If you have spine compression, this can aggravate the dormant virus to outbreak and reappear at the surface. This can include triggering hip/buttock, thigh/leg pain in addition to any genital/anus flares.
You may want to consider regular suppression daily to minimize outbreaks for a period of time to see if it helps reduce your painful symptoms.

When was your last lumbar spine and hip/pelvis MRI? You may want to see if you have any compression of spinal cord/nerve roots, inflammation or soft tissue injuries in addition to suppression medication daily.
