ADT, Brachytherapy, IMRT...done!
Just thought I'd share. Started this journey in spring 2024. Gleason 4+3. Decipher 0.78. PSMA scan negative outside of the prostate lesion. PSA 13.4 when I started Orgovyx in early August. Testosterone 582. LDR brachytherapy in early October followed by twenty-five sessions of IMRT (completed in early December). Finished six months of Orgovyx last Thursday.
So, blood panel today. Testosterone up to 5 (wow, it more than doubled, last November, and I presume last Thursday, it was less than 2.5 😁). PSA 0.06. The rest of the blood panel was good, lymphocytes excepted (which is normal post radiation). Will recheck in one month, then every three months thereafter for two years.
Now it's a matter of watching, waiting, hoping and living.
And I forgot....shoveling snow.

Your numbers look pretty good to this layman.
For reference, at age 73 following Salvage treatment of 37 IMRT sessions, 66.6 gy of radiation, and 4 mos Orgovyx, my 1st tests for PSA was 6 mos from completion of radiation; and for T, 5 mos post Orgovyx and my T was about 50% pretreatment; recovering thereafter to about 75 - 80 %. uPSA has been undetectable (had RP initially) for about 1 year.

One month after completing ADT my testosterone is 398, PSA is 0.42. The month to month improvement in the way that I've felt since completing radiation and ADT is quite noticeable. I suspected the testosterone recovery. My two pals "Bert and Ernie" are back (I now have testicles again). Fatigue is pretty much nonexistent. Some nights I'll get up to pee a time or two....some nights not at all. Bowel function is normal.
So, the recovery process is a gradual thing. Improvement can't really be assessed on a day to day, week to week basis mainly because of the short term ups and downs that are part of the process.

Blood panel last week. The most recent numbers (six months post radiation treatments and five months after completing ADT) are: testosterone - 298, PSA - 0.42. Rest of blood panel relatively unremarkable. Overall I feel pretty good. Still trying to knock off the last couple of pounds of accumulated blubber around my middle. This has been hampered by recent hernia surgery. I hope to get back to being my normal skinny self by late summer.
What's the treatment plan now?
Follow up PSA and blood panel for two years.
So no adt for now? I can’t wait to get off it but i still have 3 months to go. Plus at least a couple months after I stop . 7 more radiations. I’m keeping fingers crossed it knocks my PSA to ‘undetectable’
