New to Osteoporosis, Start here! 2025 World Congress on Osteoporosis

Posted by Michael Lavacot @michaellavacot, 6 days ago

Hello Mayo Clinic folks,

I just wanted to let you know that I posted an updated "New to Osteoporosis" presentation https://youtu.be/OVRiiyHXeh8 with updates from the 2025 World Congress on Osteoporosis that just happened in April. There were a few new trials and studies in the last year that were significant. I also updated my "Taking Prolia for Osteoporosis" training as well https://youtu.be/hnj1tFUAK3M .

If you don't know, I started treatment for osteoporosis two and a half years ago and have been doing research ever since. So this is me trying to help others based on my learnings from vast amount of trainings and studies. I do not get paid for anything here.

As always, your feedback and questions are welcome. I hope everyone is doing well.

Any news on lower doses of Reclast . I found a study that shoed takingn5/4/3/2/1 lower dose plus a control group taking none and the results were similar. Also the infusion took longer and slower. I have really bad reactions to many meds and I do not want to take Reclast at the high does of 5 becasue if you get the bad reactions there is no way to get rid of it. AND they implied this was a one time thing but after reading it can be 2 oe 3 years

Hi @hedynevada01 - There were a few presentations on Reclast (zoledronic acid / zolendronate) but not one on reduced doses. That said, there was a new 10-year Reclast trial that just came out this year that showed performance of using Reclast just once every five years. It was very effective to maintain bone density. I discuss this trial in the video. Attached is a snapshot and here is the link to the trial information https://www.nejm.org/doi/full/10.1056/NEJMoa2407031 .

I did a quick ChatGPT search and it does look like there are many trials at lower dosages with good results. Here are the results from ChatGPT https://chatgpt.com/share/684cab68-f2cc-8008-b598-4cf2b33c31d1

Was there any news for someone who cant take any medications for osteoporosis to switch to
calcium aspartate anhydrous, which is said to increase bone density instead or calcium carbonate, or to take one dose of one and the other dose of the other in the middle of the day. Thank YOU

Was anything presented about approaches that are not big pharma?

Thank you. I am beginning to learn too and will view this link.

Nothing specifically on calcium aspartate anhydrous but this might help. Assuming no "medications" can be used, the best things you can do to maintain your bones are; maintain your 25(OH) vitamin D3 levels between 30-50ng/mL, ensure you are getting 1200 to 1500mg of calcium per day (total food + supplements and not more than 500mg at any one time), eat a healthy Mediterranean diet with a good amount of protein, and do weight bearing exercise. The type of calcium (citrate, carbonate, aspartate...) does not really matter as long as it's taken with food. It all breaks down very efficiently to calcium ion when taken with food. There were presentations this year on diet and the need for protein being very helpful for bone health.

BTW, not sure if you consider taking bio identical estrogen (estradiol, the same stuff your body used to produce) as medication, but it's great for bones.

Hi @zhaner - Yes, this year there were a few presentations on diet and protein. The key takeaway was Mediterranean diet and get sufficient protein in your diet. The presenter on Protein made the argument that it was more important than ensuring proper calcium levels, but I would focus on both 🙂
