Severe Back Pain with Stage 4 Prostate Cancer
My dad (62yrs old) was recently diagnosed with stage 4 prostate cancer. PSA60 Gleason 9. It has spread to his bones specifically his back which has caused him to be in a great deal of pain. He is describing it as an 8-10 most days. He has started hormone therapy and will start Chemo next week. Doctor has prescribed him with muscle relaxers and pain medication to help but it has not help and only gotten worse. What are effect treatments that you have seen in similar cases like my dad to help relieve the crippling back pain he is experiencing .

Chemotherapy can reduce the amount of cancer in the bones and ease the pain that people have from it. I also was diagnosed at 62, Fortunately, I was a Gleason seven with BRCA2 and still alive at 77.
I know a few of people with Gleeson nine that have been around for decades after treatment.
He should probably not only be on ADT, but he should be on an ARSI like Zytiga or one of the lutamine’s. Those drugs can also shrink the cancer and relieve some of the pain.
Hello. My husband is going through the same pain. Percocet is brining the pain down to about a 4. He starts palliative radiation on Monday to hopefully get some pain relief.
I prefer 4mg dilaudid. Had my doctor give me that for knee and hip replacements. A bit stronger than Percocet but the pain sure doesn’t keep you from sleeping, or getting around.
I'm so sorry to hear that. Big question: is the cancer oligometastatic (just a few, well-defined metastases) or polymetastatic (spread throughout his bones)?
If the cancer is oligometastatic, the best/latest standard of care is to radiate the individual metastases, which has had excellent success (that's what I had, and I'm still in full remission nearly 4 years later). If it's polymetastatic, then chemotherapy, as you mention, would be likely.
In either case, combining a so-called "-lutamide" (like Enzalutamide or Apalutamide) with the androgen-deprivation therapy (like Lupron or Orgovyx) can also slow or stop progression.
I had severe pain in my leg at the beginning of my journey. Pain meds did nothing for it. About 6 weeks after starting the Firmagon and Chemo treatment, the pain subsided. Two years later, the pain has never come back and I continue my treatment meds. I understand the level of pain being so high. Tell your dad to hang in there, it will get better. I may at times complain about side effects of treatment but I will never trade it for the pain before treatment. I wish him the best on his journey. Best to all.
I too am stage 4 PC. After chemo therapy (I only did 4 rounds out of 6 prescribed) my low back pain increased to the degree of immobility. I ended up having back surgery in the middle of chemo therapy. Now have a couple of rods and screws in my back and no longer have sciatica pain. I’m not exercising ( treadmill and elliptical 4 times a week). I am currently on just ORGOVYX and have had no cancer activity since 2019

I am stage 4, 71 years old, diagnosed 11/20. My first major symptom was lower back pain. Radiation helped ease the pain a lot. My bone cancer is mostly in the back/spine. Three things help me a lot deal with the pain. 1, Floating in a swimming pool, or sitting in a hot tub. 2, Heating pad on my back. 3, Chair yoga for cancer survivors.
Also, slow intentional movements
