Docetaxel and Carboplatin

Anyone received both of these chemotherapy drugs for prostate cancer and if so how’d it work?

@lag

My husband had six rounds of docetaxel in 2016 after new spinal lesions started cropping up too fast to treat them only with spot radiation. He had the usual chemo side effects, which were at their worst in week one after the infusion, and which got better over the next two weeks--until it was time for the next infusion. He had no serious side effects such as the ones described by sillyoyster above, and the docetaxel reduced his PSA to undetectable for a long time. Everyone reacts differently to cancer medications, so do discuss the pros and cons of docetaxel with your oncologist, and don't dismiss it out of hand. It is very effective for many men.

How long did it reduce his PSA? Quality of life afterwards? Thanks

Thank you for your reply. I too never had any of the severe things mentioned above, and I also received Carboplatin. A lot of stage four prostate cancer patients have received Docetaxel But few have received Carboplatin with it, which makes it tougher and those were the people I was wanting to hear from.

It was years ago, but my recollection is that it kept his cancer suppressed for quite some time. His quality of life was good once he was a few weeks out from the sixth round of infusions.

@jajaime22, I can imagine that you are concerned that your PSA is rising after 5 of 6 chemo treatments of docetaxel plus Nubeqa (darolutamide). What has your oncologist suggested as next steps and options?

Concretamente me dice " terminemos todo el esquema , ósea las 6 sesiones y analizamos resultados : hemograma y otros indicadores , ademas acompañado de un PET SCAN con PSMA y observamos detenidamente lo pertinente para orientar opciones a seguir

Commenting as spouse of StageIV PCA with metastasis to lung only and getting recommended Docetaxel + Carboplatin. I have messaged with the OP, but wanted to continue the thread here as others may fall into this treatment going forward, given PSMA PET scans are picking up spots earlier and in visceral locations.

We are 3 rounds in on the double chemo mentioned above. Also, started Firmagon 3 days prior to 1st chemo. PSA was suspiciously low at 0.36 and after 1st 3 weeks of tx it is < 0.01, or undetectable. I wish we had the ADT for more time to see what its impact was prior to chemo (a thought for anyone pondering).

One thing I learned on this Round 3 last week is that while the dosage of Docetaxel stays consistent and is related to body surface area (ie, stable weight), the Carboplatin dose is related to kidney function, ie its ability to clear the Carboplatin (why you drink a ton of water!). So, on this 3rd round, hub's labs showed better kidney function and the Carbo dose was increased 25% - we were told because kidneys will clear it. But, this round hit the hardest of all - not sure if it's the higher dose of carbo or the expected worsening with each round - perhaps, both.

Neuropathy: We have been icing hands and feet during treatment and believe that has kept the peripheral neuropathy away. Also, sucking ice during the infusion. The steroid helps the first 2 days and then it hits - by day 6 he starts to come back to life. Appetite ok, except the 2 worst days. Other than extreme fatigue, I mean extreme, and the impact on blood counts, we are managing ok. Repeat scans next week.
