breadmaker,
thanks for the reply. I'm 69 YO. Gl was 3+4 25% 4 in one of 12 cores. no lesions anywhere else. MRI PI-RADS 2, fairly benign #'s but my Decipher was high, .80.
Do you have any experience with HIFU? with high decipher?

thanks

Ah, sorry didn't see your post till now.

Yes, your numbers - outside of the Decipher - are reasonable.

Like you, I had one core of 3+4. A bit worrisome but everything else for me has been favorable - PSA is "age appropriate" and steady. MRIs have been good. Decipher result was low risk. I also recently did the Promise study (for inheritance DNA) and that was also good.

I don't have any experience with HIFU but I see lots of posts on that and TULSA-Pro.

Having 3+4 is a challenge since it indicates some aggressive cancer is present. I've decided to stay on Active Surveillance for now - though I see my Urologist for my PSA test in two weeks.

All this is tough since on the one hand you don't want to over treat but on the other you don't want this cancer to metastasize.

We all have to do - with our medical team (we're not doctors here) - our own risk assessment.

Check out ancan.org if you are interested in a virtual support meeting where you can talk to others. They have four groups for PCa -

Active Surveillance
Low & Intermediate Risk
High-Risk, Recurrent, Advanced
Under 60

In the AS group, it's just about AS - so, no discussion on any treatment. I'm in that group.

If you think you're going to go for treatment, then the Low & Intermediate Risk would be the group for you.

All the best to you.
