I’m looking for advice on what testing should be done next. I just had an episode that started with severe right upper quadrant pain. I do not have a gallbladder and I don’t drink any alcohol. I’ve had this pain before but always subsided within an hour. This time it was over 12 hours till I was able to get relief at hospital. My alt was over 1600 and ast over 1100 and high ferritin level. Within one week they were back to normal with no medical intervention other then pain control. I still have pain but not as bad. I’ve had all sorts of testing done. So far mild fatty liver and positive Ana. I do have an autoimmune disease already. I’m waiting for a fibroscan to be done. Things that were mentioned were either lupus or autoimmune hepatitis. My doctors don’t have any clue other than something acute that happened to my liver. I am stressed out having no answers. Anyone have a similar situation and any testing that could be done to figure out my issue. I’m concerned if I continue to have these episodes it will do damage to my liver.

Welcome to Mayo Clinic Connect, @reppil63. Sorry to hear you are facing this health challenge. Makes sense you'd want to figure out these episodes.

Are the doctors going to do any treatment for the findings thus far, or are they waiting till all the testing is in? When will you have your fibroscan?

Thanks for the reply. So far no treatment. My fibroscan is this Friday. I’m waiting for a call back from my rheumatologist to see if he has any ideas. If I don’t get any answers I may ask for a referral to Mayo.

Liver biopsy as soon as possible.
My doctor messed around for a year and now I am in stage 2 auto immune hepatitis.

Good luck.
