Yes you will need a referral to see a neurologist that uses Botox.
Don’t go to a plastic surgeon the neurologist will know where to inject the Botox and they know how to word their medical reports for your insurance.
Botox is once every three months and as for me I’m on Medicare and the copayment is $250.00 per visit.
My primary physician referred me to the migraine specialist who happens to be a PA and her husband is a neurosurgeon.

Yes, I was referred to an orthopedic doctor who also uses Botox injections.
