The slight swelling is worrisome. Any fever at all?
Everything else sounds like a cervical spine issue. They need to do an MRI of your cervical spine as this will show pinched or irritated nerves, or any lymph node issues I believe.
Anytime you have difficulty swallowing, pain in the neck and shoulders my thought process goes to either cervical spine issue OR throat infection/esophageal cancer. Not to alarm you.
How old are you?

Did you get an ultrasound or MRI of your neck/cervical spine? If you had X-ray, it shows bones but not soft tissues.
It would be good to have your thyroid, lymph nodes and cervical spine checked.
