You seem to describe a stress test that a doctor would run to see if there is a problem with the heart. Wouldn’t blood pressure that high be indicative of the fact that something is wrong and treatment is needed.

No, the opposite. You don't want stress. Unlike diastolic (which changes more gradually), systolic pressure is highly volatile and sensitive to physical activity. The guidelines are for resting BP, not BP under stress.

That's why the doctor or nurse often has you sit or even lie down for a while before taking your blood pressure, and some have machines that can keep taking it every minute or so a few times. I've seen my systolic drop 20+ mmHg after a few minutes resting and breathing slowly, while the diastolic will be much more steady.
