Hello @daisyduke55555, Welcome to Connect. I'm one of the fortunate ones with only numbness and some tingling with my neuropathy but it's still aggravating to say the least. Really sorry to hear that the gabapentin alone doesn't relieve the pain but glad you've found something that helps.

If you haven't tried the search feature on Connect, it's really helpful in finding discussions that may help answer some of your questions. Here's a search of Connect for "neuropathy pain" that shows the different discussions and member comments - https://connect.mayoclinic.org/search/discussions/?search=neuropathy%20pain%20.

How long have you been diagnosed with neuropathy?

Do you have diabetes? Do you know the cause of your neuropathy? Were you diagnosed with small fiber neuropathy with a skin punch biopsy? Did you have an EMG/nerve conduction study of lower limbs by your neurologist?

EMG/nerve conduction studies are done by a neurologist to test peripheral nerve health in arms/hands, legs/feet. I think of it as how well your muscles receive communications from your brain through your spinal cord to your nerves. This test can determine if there is large nerve fiber damage impacting the signal to muscles.

Small fiber neuropathy is tested best by the gold standard of skin punch biopsy. They look under a microscope to determine if there is small fiber nerve damage.

Small fiber neuropathy can be caused by diabetes, drinking alcohol, vitamin deficiencies or toxicities, chemotherapy, etc. if they can’t find a cause, it is classified as idiopathic which means they don’t know the cause. Treatment is usually treating the cause, if possible, to stop or slow the progression and medication or supplements (like gabapentin or alpha Lipoic acid). Symptoms can include pain, burning, tingling, pins and needles feeling, numbness, etc. that can start in your feet and travel upward. Small fibers can affect temperature and skin sensations and peripheral/autonomic nervous systems (arms/legs/hands/feet and organs).

I have neuropathy and CRPS in my both my feet. I'm on pregamblin, duloxetine and Hydrocodone for the pain. It helps some. I also use Tiger Balm patches at night so I can try to get some sleep. I found they help me more than any other topical patches or lotions I've tried

My Google search said our nerves are coated in a fat called MYELIN.
Myelin is continually being replaced in the healthy body. Alcohol stops myelin regeneration because it interferes with vitamin B12 absorption. from a poor diet. By taking 10,000 micrograms of the best absorbed B12 known as METHYLCOBALAMIN nerves throughout the whole body get their insulation renewed. The healthy body only needs 5 micrograms. The high dose is simply urinated daily. Harmless. Urine will turn green.
For my trigeminal neuralgia I have cut back to 250 micrograms daily. No more pain. Great
Good luck.
