I assume everyone here talking about changes had thier prostate removed? Taking hormone blockers? Please clarify. If removed was it Robotic and nerve sparing? Has anyone seen these drastic changes after Brachy only? Trying to determine my direction with early stage confined to gland cancer. PSA 4.5, 2 of 12 with 1mm very slight 3+3=6 and area of concern having 10mm 3+4=7 with only 70% of core showing abnormal and only 10% of that is a 4 so very early. PSA PET showed no other spread. Having a hard time deciding. Any advice would be appreciated.

Your age is very important, as others have said. Treatment outcomes for both modalities are pretty much the same.
If you only have a very small area of G4, depending on where it is in the gland, you might be a candidate for focal therapy - HIFU, TulsaPro, etc.

I had SBRT and six months of ADT that ended in October 2024 but my T still hasn't recovered so maybe there's still room for improvement. I've noticed doctors like to say "let's wait six more months and see what happens" as it gets you out of their office and they don't have to deal with you for half a year.
