How Concerning is this?

Jeff, I know SBRT vs salvage IMRT was discussed on one of your recent webinars; I believe Dr Scholtz recommended SBRT?
My concern is this: If you have a visible metastasis on a scan doesn’t that mean that you MUST have micrometastases elsewhere? If you just zap the visible issue how does that treat the rest of the problem? ADT for life?
But perhaps my statement is wrong and a metastasis to a rib doesn’t mean you have them elsewhere? Dr. Scholtz’s approach seems totally counter-intuitive to me. Thoughts?
Phil

Well, you do have a point. Dr. Scholz does feel that just doing SBRT to zap metastasis that come up is sufficient for long Life. If you listen to his discussion about this on the YouTube video for the March PCRI conference It may clear up some of this uncertainty.

They also said during the same PCRI conference that when people had salvage radiation, it was only successful in 1/3 of the cases.
