Primary Sjögren's syndrome (pSS) can have elevated free light chains. Is your hematologist for MGUS or some another condition? Sjögren's can increase one's risk of lymphoma. PSS can have many effects beyond dry eyes and mouth. I have IgA Kappa MGUS, my kappa free light chain (FLC) levels are around 300 and kappa/lambda ratio is 28; these are way above baseline but I am still asymptomatic. So, it depends partly on how elevated your FLCs are and also which ones.
