I had radiation and ADT, not surgery, but my experience is similar to yours, in that it takes a long time and lots of stimulation to get "over the hump" and have an orgasm. I think radiation has done some nerve damage around the prostate and possibly other tissue in the way. I am still hoping my testosterone level recovers soon, because I think that is part of my problem. I had my last Elegard shot about a year and a half ago, T should have come back by now. My urologist gave me a prescription for Chlomophene to stimulate my own production, but it is working agonizingly slow. If it doesn't happen soon, I will again ask the urologist to prescribe TRT. I just turned 74, gleason 9, decipher score 0.92, two six-month shots of Elegard.

I am 72 and had RP at 68, Triple therapy a year later and pelvic radiation after that. PSA undetectable for last 2 1/2 years. I was on Lupron for 1 year but my T never recovered after one and a half years. Since the RP in 10/2021 up until 10/24, I had no spontaneous erections. Used the pump and Trimix.

At my last MO appointment in 10/24 at Hopkins my MO said my T recovery was too slow and wanted me to go on TRT under the direction of an endocrinologist. I made an appointment at Emory (live close by) and the endocrinologist suggested Clomiphene to stimulate the pituitary to produce FSH/LH to induce the testicles to produce T. Before starting treatment he ordered labs. My FSH/LH were high, indicating my body was already trying to stimulate my testicles to produce T but that factory was shut down. Diagnosis of primary hypogonadism-not treatable with Clomiphene. So, I was started on TRT, Androgel. My T immediately came back to normal and the endocrinologist had to titrate the dosage to get the T at the level he wanted (300-500).

I realize the physical and psychological effects of T deprivation are different for men. For me, the very low T (< 3) from my Lupron treatment was almost intolerable. Fatigue, hot flashes, loss of libido, muscle loss, genital shrinkage etc. After 7 months on T I have regained my energy, hot flashed are gone, my libido is back (!!), I have gained muscle and lost fat, my body hair has returned and I have had some return in penile length. After around 2 months on TRT I began to notice a return of nocturnal erections and erections with foreplay with my wife. Still not 100% but getting better. My orgasms are stronger than before TRT. Unfortunately, I have climaturia (ejaculation of urine with orgasm) but there are ways to help ( empty bladder before sex, limit fluid intake a couple hours before, cock ring, condom). Actually, the ejaculation of urine is a similar feeling to semen but.....it's still urine.

I have largely come to accept the new normal regarding my cancer and its treatment side effects. It's hard sometimes but I try to have gratitude for what I still have.
