Completed EBRT recently, first blood test readings done. Any advice?
I was a Gleason 9 when I began taking Orgovyx in September 2024. Testosterone was 825, PSA 6.0 Had 28 sessions of EBRT which concluded in early April 2025. My recent blood lab results were testosterone less than 10, PSA 0.08. Some undesirable other blood results: white blood count 2.6, red blood count 3.94, BUN 32, ALT 58, AST 38, hemoglobin 12.5, absolute lymphocytes 322. My radiation oncologist said most of these results were caused by EBRT and orgovyx and will slowly improve. Hope he's right. Any advice or comments? Thanks!

Were you able to get PSMA PET scans before EBRT to show locations of any metastases? If show, was the EBRT used to treat all identified sites?
In general, the larger the treatment area, the more affected on blood counts.
Everyone responds to Orgovyx differently, but your genitourinary medical oncologist should be able to advise you of the expected impact to your blood counts from Orgovyx specific to you.
The effect from SBRT on your PCa cells takes more time and you should expect a nadir (low point) after 6 months from the date that SBRT treatments ended. My nadir was closer to 12 months.
Hopefully you are enjoying being completed with the EBRT treatments and the regained flexibility on food and drink timing.
My other bloodwork has improved over the years since spinal surgery and radiation in 2021–22, even though I'm still on ADT + Apalutamide.
Early on, a number of the results were borderline; now they've nearly all trended back into the green (except testosterone, of course) as my body has adjusted to its new reality. Even my iron levels are OK again.
Your mileage may vary, but radiation, cancer, and hormone therapy are all big shocks, and your body needs time and care to recover.
I’ve had surgery, radiation ADT for 8 years And my blood counts for RBC, hemocrit and hemoglobin have all been below the minimum for at least a year and a half. My white blood cell count is right in the middle of normal And ALT has never been anything but normal. I’ve noticed no issues from having the low blood counts, And they are not going up over time the Darolutamide may be the fault right now.
So your results may vary, Hopefully over time yours come closer to normal.
My absolute lymphocytes result of 322 when the lab lists the reference range as 850- 3,900 does not provide me with much comfort.
Congrats on completing your RT! I was diagnosed with Gleason 9, cribform and EPE in Nov 2021, began 24 months of lupron + abiraterone in Feb of 2022 and completed proton therapy in July of 2022. My blood counts were very similar to yours following the completion of the proton therapy. In the year since the completing ADT, my blood counts have largely returned to the pre-treatment levels and my PSA has been < 0.03 since June 2022. Each of us responds to RT and ADT differently, but from what I read and was told by my RO and MO, it is reasonable to expect the blood counts you mentioned to improve as time from treatment increases.
