I'm 79 with 3+4 Gleason (and a 3+3) but only 2 out of 12 biopsy samples showing cancers. PSA was 7.67 October 2024 but was 4.6 in May 2025. Active surveillance for now, but the latest PSA was a (pleasant) surprise. Scheduled for another MRI and biopsy in early 2026. Anyone else have PSA declines for no apparent reason?

I’m 61. PSA was 8.1, then 7.8 two days later than two months later 5.4 in March. Did you check your Free PSA? My MRI was PIRADS 3 with no focal lesions. Doing another PSA in June and probably another MRI if numbers go back up. Maybe a benign issue. If numbers go way up and MRI notices a change then probably biopsy.
