I’m at a loss. After years of perfect sodium levels, my husband’s levels came in low. We went to hospital, the raised them with saline, and sent his home. A week later, levels were checked again. This time they monitored, and noticed they kept going down after sodium IV, so they gave him 15 mg of Tolvaptan. With prescription of 2 days a week. Levels were good for two weeks. Went to a nephrologist appt. He prescribed urea powder 4 times a week. This because you have to jump through hoops to get your insurance to approve Tolvaptan. Checked labs in 2 weeks and again level was down to 117. Went to hospital raised them and gave him Tolvaptan daily. Sent home with instructions to take Tolvaptan 3 days a week. After taking it a total of 5 days he had his blood tested again, and his levels were 118. Back to the hospital. Gave him IV and a few hours later two 15 mg. Of Tolvaptan. He is now here a total of 4 days, being given one Tolvaptan daily. They take his blood every 4 hours. His levels keep going up and down (by one or two points),. At one time they were up to 131, today they went down to 125. Doctor says he does not know what’s going on. He’ll add an IV for a while. I’m so worried and confused. Open to any and all ideas or suggestions or even comments.

How did this work out for your husband? This is going on with my father and has been since march. He’s been on tolvaptan 30mg daily for two months and his sodium is dropping again. Labs are showing his sodium staying in the mid 120s. He’s in the hospital with it at 124 still receiving 30mg tolvaptan. They tried adding salt tabs with the tolvaptan and it has only increased 1 number.
