Welcome @ron71, That sounds awful. If I understand you correctly, the big toe doesn't hurt at all when you are wearing your shoes. Have you seen a podiatrist or other foot specialist? Did you injure the big toe?

no no injury to toe, yes on podiatrist, I have always went thru house barefoot, so he suggested me wearing crocs around the house which helps but still would like to know what issue it is, especially since I am a diabetic

No no injury, it just started up about 3 months ago!! Have to always wear my cross or shoes in house or have intense pain in big toe , side of toe inside !!!
