Finasteride and HRT

Anyone know why WPATH recently listed Finasteride as "not recommended?"
I'm 28 months on HRT (Estradiol, progesterone and spiro) and noticed in the latest version of WPATH standards of care that Finasteride is not recommended, but with no reference to why. I've been on it as an alternative to Tamsulosin for BPH.
Anyone have any information?

Hi @bobbie4481, this must be confusing. Have you been able to ask your medical team what treatment would be best for treatment of benign prostatic hyperplasia (BPH) and not be contraindicated for successful transition? Might another treatment for BPH be possible for you?

Been down this road with my primary care physician. I was on Tamsulosin for years without a problem. When I began HRT, I experienced drenching night sweats. Switching to Finasteride stopped the night sweats. Being on spiro has exacerbated an incontinence issue. Met with a local urologist and am scheduled for a bilateral orchiectomy in April. I've been accepted into the U of M Gender Services process, have completed orientation, letters are all in and final consult is likely in April with vaginaplasty scheduled for October or November.

I’m sorry that you are having troubles with hair loss.

I began regrowing hair on my scalp two years on hrt. Primarily after my testosterone was sufficiently suppressed.

Post surgery I had rapid hair growth, however I still maintained a receding hairline.

In the last year…I have been taking oral minoxidil and am also using prescription nizoral shampoo. I use it everyday and rub a little retin-a on my temples and across the front of my hairline.

I’m almost looking at a full head of hair in a feminine growth pattern.

This has worked for me. Maybe a dermatologist could help you.

Oral monoxidil is the ultimate drug for hair loss.

Hair loss has never been an issue for me. The finasteride was as a replacement for Tamsulosin (prostate issues) which was incompatible with my HRT. Hair growth was simply an interesting side effect. I had the orchiectomy four weeks ago and deleted spiro from my HRT regimen, as a result. But I will remain on finasteride for the foreseeable future unless the prostate becomes cancerous.

Here's some first-hand experience. I've been taking 1 mg finasteride daily for more than twenty years. Insurance does not cover its cost, but the generic is very inexpensive with GoodRX. While everyone in my family is bald by the time they are 60 years old, I have a full head of brown hair at 75 years old that is even thicker than it was when I was 30 years old. Well, now it's starting to turn gray on the temples. The downside is that it has made getting an erection more challenging; on the other hand, that might also be because of age and lack of arousal. Viagra (or the like) just give me a headache.
