Does a rising PSA in a older person sometimes happen just because they are getting older so they can't fight off the cancer even after having a radical prostatectomy?

Unfortunately, probably not. If you have no gland, something else is making PSA.
It could sometimes be benign tissue left behind in a nerve sparing procedure so the PSA velocity would be lower.
However, if it’s steadily heading up at a faster rate (perhaps by .02-.03 or more each time) it’s probably cancer.
