After a 15 year hiatus being clear from elevated PSA and a Prostectomy, now dealing with Bone cancer metastases. Anyone else been treated for this?

Yes. I had the L4 on my back zapped over a year ago. I know a lot of people who had this done. It is normally successful. I’ve been undetectable for 17 months since then, which is comforting after 15 years. Sometimes you will get another one and you zap that. This was discussed in the PCRI conference last month. Dr. Scholz felt this had the best chance of getting you a remission. You can watch that video on YouTube if you want to see it. He talks about it near the last hour and a half.

Welcome to the club within a club. In my case, we discovered the bone metastasis first, then the cancer.

I'm not a doctor, but as far as I understand, the latest/best standard of care is something like this:

1. Just a few metastases (oligometastatic), and PSA still responds to hormone therapy (castrate-sensitive): external-beam radiation (like SBRT) to the individual metastases, and ADT (like Orgovyx) with an ARSI (like Apalutamide) to keep the cancer dormant — that's been my treatment for 3 1/2 years.

2. Many metastases (polymetastatic) and/or castrate-resistant: Pluvicto to address the individual metastases, chemotherapy to back that up, and ADT (still) to control any cancer that is still hormone-sensitive.
