Orgovyx as lifelong treatment

Does anyone who has Advanced Prostate Cancer been prescribed Orgovyx as lifelong treatment as I have?

How are you doing now @myash ? I started on Orgovyx March 26. I am not experiencing any of the side-effects yet.
After how many weeks in Orgovyx did you experience a side effect? I am concerned about dealing with the SEs (hot flashes especially) during my cruise with walking tours (excursions) on May 12 - 19, approximately my 7th week on ADT.

I’m into my third month and get several hot flashes a day. They are warm but bearable. Did my first radiation treatment this morning. 27 more to go.

That's correct about Medicare but with a caveat: Orgovyx has to be a covered drug under your Medicare drug plan. The $2k out of pocket max only applies to covered drugs.

I have Medicare with an Advantage Plan. Orgovyx is not a covered drug for me. For this year, 2025, I switched from Lupron (covered) to Orgovyx and it cost me $861 for each January and February. My March and April shipments do not cost me anything. This is part of the $2K cap for the year. Who know what will happen for 2026, with this current administration. I can't imagine a lifetime of this medication. My sentence was for 24 months with 10 months down already. New oncologist thinks MAYBE I can stop at 18 months if PSA stays down.

Well are you sure it's not being covered? I think it is. The figures you provided sound like your copay because the actual cost of the drug is $2,800+. So I'm guessing that with your March refill you will reach the $2k cap.

I had prostatectomy in 2014 and I since then, I had 2 radiation therapy done, one after 6 months of the surgery, and one 6 years after. The radiation treatment was done because my PSA level was rising but not very much. But my PSA level was rising so fast since 2019 and in 2022, my PSA was 3+ and then was 5.5 in the middle of 2024. When I was called by my Hematologist to start ADT, he said he I can start with Lupron, like getting injection every 3 months. But I did some research before I went to see him and I read online about Orgovyx, that it is a better alternative than Lupron. So, I suggested to the doctor that I take Orgovyx. I started taking Orgovyx in October 6 last year every day and my PSA level dropped from 5.5 to .8 after 19 days, then dropped to .2 after another 20 days. My last lab work last month shows my PSA level is .1 and I think Orgovyx works for me. As to the side effects, it’s true that I had hot flashes but kind of just feeling warm and not even make me sweat. The thing I observed is it makes my bones weak somehow. I had X-ray and MRI last month and the results are that my spine/hips are really kind of showing low bone density, so I may have a bone density check to get a schedule of. But taking Orgovyx while traveling, I don’t worry of the hot flashes effects.

I recently started Orgovyx. I was encouraged to take Calcium and Vitamin D. (I have read elsewhere that Vitamin K2 M7 guides the calcium to bone more so than to the cardio vascular system. Those on blood thinners should not take Vitamin K without talking with medical oncologist.) Strength exercise (resistance) helps mineralize bones.

STAMPEDE trial showed that metformin, a cheap drug, mitigates side effects of hormone deprivation therapy without affecting its therapeutic effect.

Those on blood thinners should not take Vitamin K without talking with medical oncologist.) Strength exercise (resistance) helps mineralize bones.

I think you are doing the right thing. Did your doctor tell you to take Vitamin D and Calcium when you started Orgovyx? I wish my doctor did because one of the side affects of taking Orgovyx as researched is causing bone density weak. And for me taking Orgovyx now for about 6 months, I have pain in my lower back and hips. Had an X-ray and MRI last month (March) and the results showed my bones getting lesser density. So, asked a friend who is a medical professional for advise and she told me to take Vitamin D3 + K2 and Calcium for bone strength.

ADT does effect bone density . I'm on ADT (Lupron now Orgovyx) and have discovered that I lost a lot of bone density. I had to plead with my RO to do a DEXA scan to monitor bone density. Finally got one and test showed significant density loss. I had to see an endocrinologist to interrupt the test since my RO couldn't. She put me on Vit D to start and will begin Tx soon to slow it down and improvement. Problem is there was no baseline measure for comparison, but numbers did indicate Osteopenia close to osteoporosis.
My advice is to get baseline measurements for PSA, T, and bone. Also, consult an endocrinologist to ensure new meds or vitamins are appropriate for your case.

The MO did not mention Vitamin K2. Apparently there 10 subsets of K2 M7 may be the best for bone deposition versus cardio vascular. The STAMPEDE trial suggests metformin should reduce side effects of ADT without affecting its good effect!
