Buckling ligamentum flavum
C4-c5 concerns
C2-C3: No significant abnormality.
C3-C4: Minimal shallow disc protrusion with spondylosis and some buckling ligamentum flavum results in minimal canal stenosis (9 mm). There is some minimal narrowing the left neural foramina secondary to uncovertebral and facet hypertrophy.
C4-C5: There is some loss of disc height. Disc protrusion/extrusion superimposed upon a disc bulge with spondylosis and buckling of ligamentum flavum results in moderately severe canal stenosis (6 mm). This has worsened since 12/16/2023. There is moderate left neural foraminal stenosis secondary to foraminal component of the disc with some uncovertebral hypertrophy.
C5-C6: Operative changes of anterior and posterior fusion evident. Interbody fusion has occurred. Beam hardening artifact limits evaluation through this level. There is moderate left neural foraminal stenosis secondary to uncovertebral hypertrophy.
C6-C7: Operative changes of anterior and posterior fusion evident. Interbody fusion has occurred. Beam hardening artifact limits evaluation of this level. There is some moderate left neural foraminal stenosis secondary to uncovertebral and facet hypertrophy.
C7-T1: Operative changes of anterior and posterior fusion are evident. Interbody fusion has occurred. Beam hardening artifact limits evaluation of this level. There is mild neural foraminal stenosis secondary to uncovertebral and facet

Yes, he want to do acdf c4/5. He’s been watching this area for a while now. He now says “it’s time” but leaving it up to me.

It appears you may need surgery c3-c5, depending on your symptoms and what your spine specialist recommends. The ligamentum flavin buckling seems to be pressing on your spinal cord/central canal.
How long ago did you have surgery c5-t1?

Pcdf same levels 12/23
He now recommends c4/5

What was the reason they had to go back in 4 years later for c5-t1? Was there an issue with the 1st surgery/hardware or new degeneration? How is your neck mobility/range of motion now? What are your new symptoms tied to c4-c5?
Do you want to move forward with the surgery? How was your recovery from the first 2 surgeries?
I had c5-c6 ACDF and now need c6-c7 done (hope to do in May). They are going to need to go in with a new incision in front because I have scar tissue from the first ACDF plus thyroid lobectomy using the same scar. I’ll look like Frankenstein when they are done with me. 😉 I am concerned that I will lose a lot of mobility in my neck with 2 fused sections.
I have dengeration throughout my spine. I had a non union of 2 of 3 levels done. I must say, I have pretty good range of motion. Looking up is sometimes tricky but overall good. New symptoms are neck pain, shoulder/arm pain and my hands get tingling numb sometimes. Surgery is scheduled 5/21. I know from experience it will only get worse.
