Just got pathology back post RP. Help me interpret?

Had surgery last week.. I was Gleason 4+5 going in. Locally advanced. This is what I can see from report: all lymph nodes negative, all margins negative, Gleason score 4 + 3 = 7 with tertiary 5 only 5%, grade group 3 ( is that a downgrade? I was group 5). Bad news (I think) cribiform and IDC. Net think better than Gleason 9 going in but I don’t know. Thx

Ok I will add. They surgery incorporated the IDC in the numbers..

I can totally understand your frustration with IDC finding and let me tell you, after 2 weeks of almost "manic" reading about that "entity", I am not much wiser for it. First - it became actually recognized as separate phenomenon in the 1970-es, but relatively recently was recognized as important finding used in grading aggressiveness and also as a factor in prognostic trends. Second- for long time it was mixed in bag together with "ductal" cancer. (IDC is intra-ductal) . Than there are conflicting studies about treatment results as well as importance of presentation of "loose cribriform" formation or "dense" ones which than have different prognostic trends. Also, as I read on some other discussion threads on the net, it seems that this feature is found more and more often in biopsies and now it rises a question if it is becoming more often present in new cases OR it is just that now it is more often recognized and thus acknowledged in pathology report.
Please take all of this just as crude compilation of tons of things that I read in short period of time and with very scrambled brain due to high stress levels. Of all things that I read I personally hold mostly onto article that was sent to me by Bill that showed better results with RP than RT for IDC-P, so you did great first step with removing your prostate completely. It also has relatively comforting prognostic curves.

@surftohealth88 thank you brother! It’s a fricken rollercoaster. I saw my downgrade to g7 and was thrilled but… thx for the support on surgery decision. I guess the benefit is you end up knowing exactly what you are dealing with. Good vibes back ! 💪🙏🤞👍

@tmestanas91, it looks like you received your pathology results before your appointment with your doctor to review it together. This can cause a lot of anxiety. Members here on this forum can help with the anxious feeling. But interpretation of the results should be done in consult with your surgeon.

Your doctor is the person equipped to interpret these results along with your medical history and any other tests you've had done.

When do you have an appointment with your doctor to discuss the results of the pathology and to talk about the next step?

I imagine I will hear from him shortly. I’ve received some general comments but nothing too specific on my results. I understand your concerns and I appreciate you reiterating them. Thank you

Hey TM91, it’s like they gave you a box of candy wrapped with barbed wire - an enigma wrapped in a riddle- very confusing and hard to interpret until you speak to your doctors.
But the EXCELLENT news is that all margins are negative and all lymph nodes are negative. What more can you ask of your surgeon, right? Regardless of what comes down the road (hopefully nothing) you are off to a wonderful start with a successful surgical outcome.
I am not an oncologist, nor am I a pathologist but if these were MY results I’d be happy! Best,
