Pancreatic Cancer: Anyone have liver metastasis after Whipple?

Looking to speak with others who have had a successful Whipple procedure, but cancer metastasized to your liver

I had a modified Whipple in 2015 for insulinomas x 2. 3 months later had more hypoglycemia - side effect of the insulinoma. In early 2017, I had a distal pancreatectomy and splenectomy for 9 additional insulinomas. 1 month later later additional hypoglycemia that persisted. 6 years later, tumors metastasized to the liver = metastatic insulinoma. 10 years later in continue to do well on Octreotide every 28 days and other procedures to keep hypoglycemia under control. I have had great care at Mayo since 2017 to keep me well! Thankful for the care at Mayo!

I had a Whipple at Mayo Rochester in October 2024 as well and my scan last week showed two lesions on my liver, 2 cm each, consistent with metastatic disease. I am getting a biopsy on Wednesday this week. Otherwise, I feel great and was recovering well from my Whipple. My weight is steady, I have good energy and my blood work is good except for some elevation in alkaline phosphatase (ALP). My last scan in December was clear and my ctDNA test in January showed no tumor DNA and my CA 19 levels in December and February were at 4 (my tumor did not produce much CA 19)
@kbower1611 how are you doing and who are you seeing at Mayo ? How is the chemo working out and what does your onco recommend as next steps ?

Hi I was diagnosed with stage 4 pNET in July of 2023, I had a Whipple and liver debulking on 31 October 2023. I have had monthly lanreotide injections since March of 2024 but my cancer continues to progress. A PET in November of 2024 showed that I have a tumor in my skull. At this point, I don’t know what to think anymore.

I had the Whipple October of 2024 and then 3 months later when I went in for blood test and CT scan it had went tmt liver 5 spots. Was like how could this happen being diagnosed with stage 1 thought every was going to be good. My team had given me a good out look before the Whipple. I did ask my surgeon if he could freeze the spot of cancer before he cut it out so it would be all contained. But he said that's why we do chemo before doing surgery to kill the cancer cells flowing around. I am not a Doctor but still makes sense to freeze it before cut it out so it would all stay contained. So had to go back on chemo doing Gemcitabine regiment had a CT scan 2 months later and this regiment wasn't working had a couple more spots grow. So now I am back on 5fu regiment. Hopefully I am going to get on a clinical trial soon. I see they have new trials coming out all the time. Hopefully one will work 🙏 I do feel the best that I have since this all started last year. Just hoping and praying for the best and 2 be like stage I V surviver. Have you tried to take creon for your diarrhea and stay away from foods that have fats in them. Stay positive! I wish all of you the best! Stay in touch it helps to see what others are doing and what is working because it could work for someone else also.

I had Whipple procedure in October of 2024 . I caught the cancer early stage 1 . My team suggested chemo first 8 treatments every 2 weeks would come home with chemo pump. Then had 3 weeks off and then started radiation and also took a chemo pill. This all happened before the surgery. Had surgery was out of the hospital in 5 days after the Whipple. I had to wait 3 months before I had CA-19 blood and a CT scan. My CA -19 blood test in September of 2024 was 190. When I had in done 3 months after my surgery it came back in the 3000's. My CT scan also showed that I have 5 spots on my liver. My biggest spot is 2.6 cm all the rest are smaller. Just had a liver biopsy and started chemo again ( different regiment) Hoping this will work. I feel great I have energy so I can't believe this is happening. If anyone out there has experience the same thing let me know. Would like to know how you are doing and what worked for you. I am keeping very positive!

My dad’s going through this right now. Whipple May 2023 , one month shy of two year remission and now likely liver cancer. One small mass. Pet scan was clear last month. The spot on the liver was discovered on an ultrasound when they found out his portal vein to the liver had a clot.

We’re waiting for the dna test to come back and see what treatment if any.

Sending strength as you go through treatment

Hello @mn86az and welcome to Mayo Connect. I am sorry to hear of your dad's recurrence of cancer. I was wondering if your dad had any other treatment following his Whipple.

How is he feeling? Has he experienced any weight loss or problems with his appetite?

When do you expect to receive the results from the genetic testing?

I look forward to hearing from you again. Will you post again with an update?

There are many liver directed therapies available, including surgery. Different forms of embolizations, ablation, histotripsy, etc. Make sure your net specialist considers all options.

@mn86az, as @lindabees posted, there are several treatment possibilities for liver cancer and liver metastasis. Did you dad complete further testing and find out what next steps are best for him?
