Advice after hip replacement with osteopenia

I had anterior hip replacement Sept 2024. My surgeon has released me from all restrictions yet there is very little research about the healing process/bone remodeling for those like me with osteopenia.

My t-score was -2.3 a week before surgery yet I have read that the prosthesis will reduce t-score, thus pushing me into osteoporosis.

I seek advice on protocol for increasing activity given the low bone density. Generally, strength training increases density, but my concern relates to the bone remodeling. I wonder if it will take longer for my bone to secure to the prosthesis and if strength training should therefore be light? My surgeon says to follow the regular protocol of those with normal density, but I find no studies on this and want to make sure prosthesis never loosens.

Thank you Kay! This is top of my list. I’m gingerly starting PT concerned that low density could mean slower remodeling into implant. When did you start increasing workouts? At what week and did you add more week by week? Sounds like you opted not to start any meds? You give me hope tho my # was nearly osteoporosis and yours sounded higher.

I had a total hip replacement two years ago and at that time I had osteoporosis. Now, I have osteopenia thanks to aggressive drug treatment.
No problems with the surgery. I agree that hip replacement is a rough procedure. I watched a video of one before having mine. I don’t advise that.
I had 100% success with mine. Fast recovery, no problems. I was aggressive about physical therapy, though.
I am able to resume an active lifestyle since the hip replacement.

Thanks for the info and encouragement. May I ask what drug you are taking that put you back into osteopenia. I agree that staying up with PT and then staying active are key.

No meds have been helpful this far. Very discouraging. I did Prolia for 1 year with no difference in my Dexa scans. Then, I did 5 injections of Evenity, then I fell and suffered my 2nd Fragilty Fracture resulting in my 2nd Total Hip Replacement. I’m bilateral. Both hips have replacements. I’m only 63, so this is very serious for me.
I am thinking of HRT next for my bones.
