Hormone Levels after 90+ days Lupron

Background: @ age 62 Radical Prostatectomy May 2020. Post RP pathology report stated high risk, confirmed 4+3 Gleason, positive margins, Perineural invasion, as well as invasion into the neck of the bladder. No salvage radiation post surgery. Three years post RP rise in PSA to 0.78. PSMA scan Sept. 2024 positive lymph node metastasis in pelvic wall. Started Lupron Nov. 2024. Tested PSA and hormone levels March 2025. PSA now at .08 however, the testosterone level is at 241 ng/dL - a low 'normal' range. I've read where the target level is < 50 ng/dL. Could this be a sign of some resistance to the Lupron? Thank you in advance.

Normally, they would start salvage radiation at .2 PSA after a prostatectomy. If you wait, a lot longer then the cancer can get out of the prostate bed, which could make the salvage radiation insufficient. Can they zap the cancerous lymph node with SBRT radiation, and also give you salvage radiation. Or is that lymph node in the salvage radiation area?

Lupron is Apparently not working since your testosterone level is so high. It should be below 20 right now. This may mean you are castrate resistant, not castrate sensitive.

Normally, with what you describe, they would add a second drug, an ARSI like abiraterone, which can bring most peoples testosterone down very low, even if castrate resistant. Another alternative if abiraterone doesn’t work a lutamide actually makes the testosterone ineffective. There are Three main lutamines, apalutamide, Darolutamide and Enzalutamide. Darolutamide has the least side effects.

I’m telling you about this so that you are prepared when you speak to your doctor and know what the possible options are.

Thank you very much for the information. Yeah, I got behind the curve ball in checking my PSA levels after the 2nd year post RP . The third year other health issues consumed me and I went a year without checking. Of course it decided to take off that 3rd year. I'll see my doc in a few weeks. My Testosterone Level is currently at 40. My last LUPRON-3 Injection was mid-June 2025.
I can honestly say that I am STRUGGLING with my day-to-day activities; very low energy, brain fog, short-term memory loss, etc.
Anyone else experience this and any solutions?

A lot of people struggle with exactly what you’re talking about. Low testosterone can be a problem? For some people testosterone never comes back to Levels that take away those side effects. It is very common for people to take a long time to have their testosterone returned to normal After they stop Lupron.

I was on Lupron for six years and orgovyx for 9 months when I stopped Taking Orgovyx. After seven months, my testosterone hit 50. It was going up about 25% a month. Sounds similar to what you’re having. I was 77 and on it for a long time. I know what the rate it was going up if I gave it another six months I’d be oh well over 100. It’s a known fact that it takes longer for testosterone to come back as people age.

There is an option. You could get testosterone injections that would boost your testosterone level. Some doctors won’t do it, especially if you had a high Gleason score. Speak to your doctor about getting injections if you can’t wait any longer. There is a risk to doing this, Your cancer could come back more quickly, so get PSA tested more frequently.

Now 6 months after stopping Lupron, my horrible side effects are just starting to lessen. I had severe headaches, 24 hour a day soaking sweats, joint and muscle pain, brain fog, and multiple muscle cramps. I am 83 and had a PSA =11.1 and Stage 3B prostate cancer treated with 44 radiation treatments and 4 months of Lupron. Had to stop the Lupron as I felt it was killing me. At one month and 4 months after stopping Lupron I had PSA < 0.1 and testosterone < 8. Several recent studies including Duke Health's AI work indicate that low testosterone may actually help aggressive cancer. It may take a decade for the health industry to determine the best hormone treatment.

The problem with low testosterone is that some of the cancer cells get immune to low testosterone and start to grow without it. That’s when somebody becomes castrate resistant. They try to use BAT Where you get infusions Of testosterone and it kills off those cells that became immune to it and sometimes you can go back to the drugs you were taking before and they work again.

So many different things going on. BAT doesn’t work for everyone, unfortunately.
