After 6 Cycle of Chemotherpay, do we need Chemo + Radiotherapy ?

Posted by doshi @doshi, 2 days ago

I’m reaching out to connect with anyone who may have experience or knowledge with Chemo + RadioTherapy Treatment.

Cancer : Endometrial Adenocarcinoma Stage IIIC, 2nos LymphNode
DNA: BRCA1/2 and HRD Negative, She is pMMR, PL53 negative and ER and PR Weekly focal

Treatment Complete: Surgery + 6 Cycle of Chemo (carboplatin+taxol)

Current Status: No decease found in CT and CA125 - 15

Next round of treatment : Planned for RadioTherpay but doctor said he is opting for Portec3 - > Cisplatin Chemo + radioTherapy. (27days of external beam radiotheroay + Week1 and 4 Cisplatin chemo)

Question: Is Cisplatin + Radiotherapy is common and is it needed or just radiotherapy is enough? As I was advise it can add mor toxicity which may not be needed due to evidence of cancer.

Interested in more discussions like this? Go to the Gynecologic Cancers Support Group.

I think you should get a second (oncology) opinion if you aren’t comfortable with the plan. This is a peer forum and not the place for a second opinion. I know cancer treatment is scary & overwhelming (from personal experience), which makes us hungry for advice, especially when the treatment is so difficult & often toxic. Have you had this discussion with the ordering physician? I’ve found them to be very open to discussing their reasoning, & the risks of differing treatment. Best of luck to you.

@doshi Welcome back. How is your wife feeling after the previous course of 6 Cycles of Chemo (carboplatin+taxol)? As I write this I am hoping that other members in our Gynecological Cancers Support Group will share their experience with radiation therapy and/or chemotherapy.

While it is true that we aren't medical professionals and cannot offer a "second opinion" we can certainly share our own experiences and make suggestions on how your wife can make her own decision. What does she say she would like to do after this change in treatment plan? Have she discussed this with her oncologist so that they can explain why this change in treatment plan is recommended?

Given this change in plan would you and your wife like to get another medical opinion?

the way my doctor explained it to me.........chemotherapy is best at treating metastases and radiotherapy is best at treating local. to me it seems like your doctor is covering all bases considering the staging and suggesting a treatment protocol that offers the best chances of remission.

@naturegirl5 Thank you. And yes I am not looking for medical advise but want to understand that After 6 round of chemo, is it standard practice to have another 2 round of chemo with Radiotherapy, or just radiotherapy?
BTW I have organised appointment with Oncologiest to discuss this but thought if get experiances of others than it will help me with meaningful discussion with him.

To my knowledge, giving cisplatin with radiotherapy after completing 6 cycles of carboplatin/paclitaxel is not standard practice. The Portec3 protocol is radiotherapy with 2 doses of cisplatin during radiotherapy, followed by 4 cycles of carboplatin/paclitaxel. Standard practice would have been to do this instead of 6 cycles of carboplatin/paclitaxel. But cisplatin does apparently make radiotherapy work better, and obviously it is too late now to do this the normal way.
