My ostomy predates smartphones, so by the time they were invented I was well past the initial stages of trying to figure everything out. I used a spreadsheet to track my appliances, and what worked and didn't work... it took a while for me to find what worked for me and I found that to be helpful.
I currently keep a memo pad with my supplies and record the dates of my changes, just so I know when I did the last change. I have all my supplies in one drawer, so when I am down to one box of bags, I do an order.
I took a quick look at what is available on Android (which is what I have). It doesn't look like anything does everything you want, but "My OstomyJourney" looks like it lets you log food, appliance changes, but not output. OstoBuddy has a different subset, and MyOstomyLife seems to be more about usage and exercise.... I would say try the different ones and let us know! I used the LoseIt! app to track food when I was having issues being underweight, and that lets you track carbs, protein, etc.
I keep my changes on ye olde bathroom calendar.....
