It's possible to get a tear in the skin chronically using suppositories. Anal fissures can sometimes be seen on an exam or not. I was diagnosed when I had a colonoscopy. I had to get a compounded cream to treat it. If it gets infected you could need antibiotics. Either way, even a tear can affect your bowel movements, fixing it could help all around. Hope you find relief.

My experience with this very real problem may be helpful. A good bowel regimen is essential for good health. I had 2 serious issues--constipation because of opioid use and peri-anal pain from arachnoiditis. It's just human nature to look for the quickest and (maybe)simplest solution for having a complete and easy emptying of the bowel.
Let's quickly jump ahead 50 years from the 1970's when all my problems started, and I've had hemorrhoids, fissures, constipation, blockage of the bowel--the whole nice yards all from the 2 main issues. I've tried all the solutions you are talking about and probably a few more. Anyway, I finally met a gastroenterologist who advised me that having an easy bowel movement daily is NORMAL, not every 3 days or more as I had experienced and prescribed LACTULOSE, A LIQUID, CHEAP LAXATIVE, that works easily and solves "butt" issues. Please talk about this path with your primary care physician.

Have you tried NIFEdipine 0.4 % rectal ointment (compounded)? My colon and rectal doctor prescribed it for me and it has helped.
My mom took this is the last months of her alzheimers journey and it didn't work for her. I myself get relief from magnesium citrate, the flavored drink. I probably use it about every 3 months. Just stay close to home. Lol Right now I'm having issues with chemo plus I've used opioids for years for chronic back issues.
