Did you have a swollen scrotum after prostate surgery?

My husband had a RLP on 2/28/2025 and his scrotum is very swollen. I brought him to the ER last night and they did a cat scan of his pelvic area to see if there was an issue with the lymphatic system/drainage and the doctors there said it looked like normal post operative swelling. In all of the posts I have read, I don't ever see anyone talking about the swelling. I am taking him to the Dr. on Monday to have the catheter removed. Will having the catheter out reduce the swelling? Please advise. Thank you!

He was in the ER on Friday night and they saw his swollen scrotum and did a cat scan of the pelvic area to see if there was a problem that went beyond normal surgery swelling/healing. The APRN will be removing the catheter tomorrow morning and I will be at this appt as well. I am going to ask her about the infection thing...You can still have an infection without a fever. I have had urinary tract infections and never had a fever with them... Also, I am wondering if he overdid it with the walking on Thursday and Friday... His medical team is saying it's normal and will just take time but I do not want him to suffer. The surgery and everything else that will be going along with it is ENOUGH without him having to deal with this. I would like to make it as easy as possible for him so that he can recover more easily.

His scrotum is no longer black and blue but it is still swollen…did you have your lymph nodes removed? Did having your catheter remove help with the swelling??? Or did it not make much of a difference?

Yes my lymph nodes were removed. The catheter removal was a big plus because life became for more comfortable. I can’t say that reduced the swelling but I could more easily elevate my legs and that helped. That tube attachment and bag were a nuisance, but a necessary one. I learned too late that I was allowed to switch the tube-holding apparatus to my other thigh. That would have helped also.

Well, yes there will be an issue with lymphatic drainage, on top of the normal swelling/inflammation that happens after any injury ( which surgery is). My whole groin area was black and blue and swollen for 1-2 weeks after surgery. Even now years later, I have more water retention in my lower legs and area of the pubic bone due to the disruption of lymphatic flow caused by removal of the glands and ducts during surgery.

Make sure you talk to the doc about it Monday. Sometimes the cath removal is done by a tech, with no doctor appearance. State your concern immediately, and ask to be examined by the surgeon.

At my catheter removal day a few months ago I discussed the same situation with the doc, and he was not at all concerned that anything was wrong. He advised me to elevate the lower body, and place a rolled up towel under the scrotum to further elevate, and to ice the area intermittently throughout the day. I did this and within 2-3 more days, things were improving. Within about two weeks post op the considerable swelling had gone down almost totally.

I have never heard of this occurring and I’ve heard from over a hundred people who have had surgery. What you describe is not normal, I know after my surgery I had no swelling at all.

Hopefully the doctor on Monday can help with this.

I had my RLP on 2/3/25. When the nurse removed my catheter she warned me that I would have swelling of the scrotum. I did experience swelling for several weeks but over time the swelling went away.

It is interesting how many people have mentioned they had the same problem. I know I didn’t, but Maybe there is some difference in surgery. After all this time, I’ve never heard of people complaining about this, Seems that it isn’t talked about much, bringing it up here, really exposed the problem.

I experienced significant swelling after surgery, which my doctor had warned me about and reassured me was normal. As expected, everything eventually returned to normal. Now, with ongoing ADT, I’m facing the opposite issue crazy. It’s certainly been a journey—one that many of us on this board can relate to. Stay positive, and you’ll get through this!
