PSA Nadir is main deciding factor when one can start on Intermittent H

I have been on orgovyx from April 1st,2024(11months). I had SBRT 5 sessions started on April 8th and finished on April 18th.
My PSA on October 7, 2024 was 0.06
PSA on Feb 27,2025 is 0.03 (50% reduction in about 4 months)

JEff,
Do you know of any cases someone with Gleason 9 or 10 and were on ADT for 24 to 36 months and still had cancer recurrence with metastasis within a year or two?

It definitely happens. Something don’t have it come back for years and others. Have it come back within months.

That’s one of those things the decipher test can tell you you’re probability of it happening

I was gleason 10. I had 45 Radiation and 18 months Orgovyx. My Dr had initially wanted 24 months. My PSA had dropped to .04 at the end of radiation, and stayed there for the next 12 months . At 18 months on Orgovyx it was decided to try a holiday, that was March of 2024. By September my psa had gone to 1.5, by Oct 24 to 2.5. I went back on Orgovyx and had a PSMA scan. It showed lesions on my spine, rib, femur. I am now on Orleada and Orgovyx. Just finished cyberknife on my rib and spine, still have the radiation coming up on my arm. Not sure if a longer stay on Orgovyx would have changed anything, but what I have now is a much bigger struggle than just being on Orgovyx.
