PSA goes up and then down
I am a 18 year survivor of Prostate cancer. I had a Prostatectomy done in 2006 and radiation in 2013. I’m currently on my second recurrence. I get a PSA blood test every 4 months. Sometimes my PSA level goes up and then the next time it goes down. It’s been going up and down for several years. Can someone explain that to me ? I am not currently on any treatment.

What is the range It is going up and down within?
When on Zytiga my PSA was as high as ,7 and as low as undetectable always bouncing .
BPH Can raise and lower your PSA?
It went from 1.18 to 1.11 to 0.95 to 1.06 to 1.34
Options to consider
Get a PSMA pet test to see if any metastasis
If you get 3 rises in a row it is usually an issue to address with the doctor
Get more frequent tests, at the least every 3 months
Your cancer appears to be very slow growing. You may outlive it doing nothing,

I did get a PSMA scan done 11 months ago and it was clear. I just don’t know why the PSA goes up and down and up and down . It’s been doing this during my 18 year journey. Im Seeing my oncologist in a week.
Still a LOT we don’t know about this disease. It could be your own immune system beating it down and then it slowly rises….rinse, repeat.
I don’t think a PSE test would help since you have no prostate and had salvage therapy as well; PSA has to be coming from cancer cells, but as pointed out, either weak clones or not very aggressive ones at this point.
Had a guy in the Ancan.org advanced prostate cancer weekly meeting last week. He had his prostate surgery 30 years ago. He has a Gleason nine. His PSA is now 170 but a PSMA pet test shows nothing.
His PSA has always been a little high, but his cancer has never seemed to reoccur. Could be yours is nothing, Or could be telling you about future problems?
So crazy nobody is the same with prostate cancer.
My Gleason is 3+4 7. And my original PSA was 5.53 when I had my prostatectomy in 2006. I wish someone would find a CURE for this stressful disease!!!
